Improve exclusions to match regular expression not just extension

I find a lot of people share files in folders I don't want. thumbs.db and desktop.ini are classics, and easily excluded by extension. But there are also more complicate filenames especially the Windows media Player generated cover images that could easily be excluded with a regular expression match. Regular expression support is native to many languages now and easily accessed, and should be pretty cheap to implement and improve exclusions considerably.
