Add disappointed user name to Shared File Not Found log

Description:
The diagnostics log for Shared File Not Found says what unavailable files people tried to queue from you, but it doesn't say who was trying to queue them. I would like to know who made each request, so I can contact them and try to help them get a file which has probably just moved to a different folder.
For example, I'd like to see each line in this log begin with "For <username>, path <path>" instead of just "For path <path>".
